2. Clinical Services
Psychological services made available through LeaveConsult.com in California are currently provided by Heal Before Work Psychology & Consulting, Inc., a California psychological practice.
Heal Before Work Psychology & Consulting, Inc. is responsible for all clinical services it provides, including psychotherapy, psychological evaluations, diagnoses, treatment recommendations, clinical documentation, leave recommendations, workplace accommodation recommendations, and patient care.
Ellimac Pipeline Inc. does not participate in clinical decision-making and does not determine whether an individual qualifies for or should receive psychological treatment, a leave of absence, workplace accommodations, intermittent leave, disability benefits, or any other clinical recommendation.

4. No Psychologist-Patient Relationship Through Website Use
Visiting LeaveConsult.com, reading website content, submitting a general inquiry, or requesting information does not by itself establish a psychologist-patient relationship with Heal Before Work Psychology & Consulting, Inc. or any other provider.
A clinical relationship is established only after the applicable provider accepts the individual for services and the required intake, informed consent, and clinical procedures have been completed.
5. No Guarantee of Leave, Disability Benefits, or Accommodations
Contacting LeaveConsult.com or participating in a psychological evaluation does not guarantee that a clinical provider will recommend a leave of absence, certify disability, recommend workplace accommodations, recommend intermittent leave, or provide any particular documentation.
Clinical recommendations are based on professional judgment, clinical findings, functional impairment, treatment needs, and other relevant information.
Approval of FMLA, CFRA, California SDI, workplace accommodations, employer leave, or disability benefits is determined by the applicable employer, government agency, benefit plan, insurer, or third party administrator. Neither LeaveConsult.com nor a clinical provider can guarantee approval.
6. Mental Health Focus

Clinical providers may have experience completing documentation requested by employers, government agencies, insurers, and third party leave administrators, including Sedgwick, Prudential, The Hartford, Lincoln Financial, Aflac, MetLife, Guardian, New York Life, Mutual of Omaha, and others.
References to these organizations are descriptive only. Unless expressly stated otherwise, LeaveConsult.com, Ellimac Pipeline Inc., and Heal Before Work Psychology & Consulting, Inc. are not affiliated with, sponsored by, endorsed by, or representatives of these organizations.
